How to Start the Diablo 4 Poisoned Hearts Quest

Diablo 4 Poisoned Hearts Aneta's Keepsake

The Poisoned Hearts quest will be available after completing A Price to Pay, and you’ll find yourself outside of the Blind Burrows dungeon. You’ll listen to Vera, and then collect Aneta’s Keepsake from the ground outside the Dungeon’s entrance to initiate the quest.

After grabbing the Keepsake, you’ll need to head east to extinguish some Strange Braziers holding Aneta captive. The Braziers are north of the Ghoa Ruins dungeon and west of the Faceless Shrine dungeon in Hawezar, and the location will now be marked on your main map.

Once there, interact with all three  Braziers to extinguish them. Then, speak with Aneta. She’ll need your help gathering four Rotten Livers.

The Rotten Livers are located in The Rustwash region of Hawezar, and the general area will be marked on your map, but the Livers’ location will remain a mystery. I found them along the outer area of the circled region, the northwest, northeast, southeast and southwest edges. If you work your way around the circle clockwise you should have no trouble at all finding them.

After collecting all four, head to the Hidden Glade dungeon. It’s directly east of your location and it will now be marked on your main map.

Once inside, follow Aneta to the end of the dungeon. You’ll need to help her fill the Blood Altar by eliminating enemies during a wave of attacks within the red circle. The enemies don’t come with any surprises, so I had an easy time defeating them.

After the Altar is full, Aneta will drink the blood. Then, a wave of angry villagers will come for you to defeat. After eliminating all of them, speak to Aneta then head back to Gulyas’ Hovel.

You’ll need to speak with Gulya, and she’ll tell you about a note that Aneta left for you on the table behind her. Read the note, and you’ll complete the Poisoned Hearts quest. I was pretty surprised that Aneta left without a proper goodbye, we went through quite a good amount of effort stopping the voices in her head.
